When you book your treatment you will be emailed a online medical screening form appropriate for your treatment and whilst we are in pandemic mode you will also receive a COVId9Please complete this prior to your treatment.

Once you submit your form you will receive a copy by email which will include location and travel information and advice on what to wear, if you are coming for injury/remedial/rehab treatment.
